At PrimeHealth+, we’re committed to providing high-quality, compassionate care for individuals and families across Mesa and Montrose Counties. As a non-profit, Federally Qualified Health Center (FQHC), we offer a full range of medical, dental, behavioral health, pharmacy, and preventive care for patients of all ages, regardless of insurance status.

Our licensed and experienced providers deliver comprehensive care, including routine check-ups, illness diagnosis and treatment, vaccinations, screenings, full-scope dental care, and counseling to help you achieve and maintain health. We accept all insurance plans and offer a sliding fee scale to ensure care is affordable for everyone.

Visit us at one of our clinic sites in Grand Junction or Montrose that’s convenient for you. Appointments are required, so contact us to schedule your next appointment or become a new patient.
