Becoming a Patient at PrimeHealth+
Thank you for your interest in becoming a patient at PrimeHealth+. Before you are able to be seen by one of our providers, we will need some basic information. To become a new patient, simply complete the following items:
- Complete the New Patient Registration Form
- Complete the Financial Assistance form. Even if you think you may not qualify for discounts, we strongly encourage all patients to complete this form because you may be surprised what you qualify for!
- Submit the forms to our office at least 24 hours before your appointment. You can do this by bringing in the printed, completed forms to any of our locations, or submitting online through our online form manager.
Don’t forget to confirm or cancel your appointment by noon on the business day prior!

Scheduling an Appointment
We appreciate the opportunity to care for you and/or your family. Whether you need a routine physical exam, help managing your diabetes or depression, or dental care, you can trust us for high quality, affordable service. Please call our office to schedule an appointment, and make sure to send in any new patient paperwork at least 24 hours prior to your appointment.

What if I’m Uninsured?
If you do not have health insurance, you may qualify for one of our discount programs that allows you to access health care at discounted rates. You will be asked to meet with one of our Eligibility Specialists to determine if you and/or your family qualify for the programs based on family size and income.
Our Eligibility Specialist will explain the details of applying for our sliding fee programs. You must provide proof of income to apply for the programs and bring your picture ID.
