Integrated Health Center
PrimeHealth+ operates two School Based Health Centers (SBHCs) in Mesa County high schools that provide integrated health care for only those students and school staff. SBHCs offer a combination of primary care services, dental services, behavioral health services, nutrition education, health education, and health promotion to students during school hours. The SBHCs are located inside Central High School and Grand Junction High School.

Our Services


Medical Services
- Visits for colds and flu
- Immunizations
- Routine labs, tests, & throat cultures
- Sports physicals
- Reproductive health & education*
- Care for chronic conditions like asthma and diabetes, in coordination with the student’s primary care physician
*Contraceptives available only for those over 18 or with parental consent if under 18. Referrals can be provided.


Dental Services
- Dental screenings
- Dental cleanings
- Varnish application
- Oral health education


BEHAVIORAL HEALTH
- Help with stress & anxiety
- Evaluations & counseling
- Crisis intervention
- Screenings and help with mental health & substance use disorders


Other
- Referrals to specialists, if needed
- Assistance with enrollment for our sliding fee discount program, Medicaid and CHP+
