Sliding Fee Scale

PrimeHealth+ serves patients who are covered by Medicaid, Medicare, as well as other commercial plans.

A sliding fee discount program is available for patients meeting eligibility requirements. If your gross household income is at or below 200% of the federal poverty level, you may qualify for our discount program as well as other services. Financial assistance for services at PrimeHealth+ begins with a phone call or a stop at our clinic to speak to our Financial Assistance Specialists.
