Notice of TriZetto Provider Solutions Incident
TriZetto Provider Solutions (“TPS”) is a third-party vendor of our electronic health records provider, OCHIN, providing services relating to patient health insurance claims and eligibility verification. On December 12, 2025, we received notification indicating limited patient information may have been potentially impacted as a result of this incident. Importantly, no PrimeHealth+ systems were impacted by this incident. Upon discovery, we worked to obtain additional information on the incident, confirm the scope of impact, and ensure all proper notifications are completed.
Based on the information provided, on October 2, 2025, TPS became aware of suspicious activity within a web portal used by some of TPS’s healthcare provider customers. Upon discovering the incident, TPS quickly launched an investigation and took steps to mitigate and contain the issue. TPS further engaged external cybersecurity experts and reported the incident to law enforcement. TPS determined that unauthorized access began in November 2024, and impacted some records related to insurance eligibility verification transactions that healthcare providers process to assess insurance coverage for treatment services they provide to patients. TPS then conducted a thorough review of the affected data to identify what information was involved and the individuals to whom the data was related. According to TPS, the types of information contained within the potentially affected data may have included your name together with one or more of the following: address, date of birth, Social Security number, health insurance member number, provider name, health insurer name, primary insured information, and other demographic, health and health insurance information. TPS has advised that this incident did not affect any payment card, bank account, or other financial information. At this time, we are not aware of any identity theft or fraud related to the use of any affected individual’s information.
We have worked to issue notification to those identified as potentially affected. Individuals will also receive a separate notification from TriZetto directly which will include contact information to obtain additional information regarding the incident and its impact, as well as instructions to enroll in complimentary credit monitoring services provided through Kroll.
