Is PrimeHealth+ (PH+) a non-profit organization?
Yes! PrimeHealth+ (or PH+, for short) is a non-profit 501(c)(3) organization. We are registered with the Colorado Secretary of State with registration #20251415857 and EIN 84-1085822. Our legal name is Marillac Clinic, Inc. (dba PrimeHealth+).
Who can receive care at PH+?
Everyone can receive care at PH+, regardless of age, income level, insurance situation, gender, sexual orientation, or any other characteristic or life circumstance. We welcome patients with Medicaid, Medicare and Medicare Advantage, or commercial insurance, and those who are uninsured or underinsured. PH+ is in-network with nearly every insurance company.
How do I become a patient at PH+?
You can stop by our main location at 526 29 ½ Road or any of our locations and pick up a registration packet, or you are welcome to download and print a pdf here. We encourage all patients, even those with insurance, to also complete a financial assistance eligibility packet. You may qualify for our Sliding Fee Discount Program, which could conserve your out-of-pocket expenses and make some services more affordable.
How do I know what I will pay for my care?
Our team will help determine what you will pay based on your income and household size, and whether you have insurance coverage. Even if you have health insurance, you may still be eligible for our sliding fee discount scale. Your circumstances determine the amount you will pay for services, making your care affordable. What is a sliding scale discount fee?
Sliding scale discount fees are the nominal fee or copayment amount for services based on the patient’s ability to pay. Fees for services are reduced for those who have lower incomes. Note that sliding scale discount fees are not health insurance. Even patients above the income scale for the sliding fee discount are welcome at PH+. Click here for our Financial Assistance packet.
Is health care at PH+ free?
Except for individuals who are homeless, our services are not free. A sliding fee discount scale approved annually by our Board of Directors enables patients to contribute according to their ability to pay. No one is turned away for their inability to pay.

If I am a patient and I have family members who also need health care, are they automatically able to receive care at PH+?
We welcome all members of your family; however, to apply the sliding scale discount fee, each adult member must be screened by our Financial Assistance specialists.
Does it matter which PH+ health center I go to for care?
PrimeHealth+ has four clinic locations in Grand Junction and Montrose, but our 12ᵗʰ Street location does not offer dental services. Once you become a patient, you may choose the location that suits your needs. Many new patients will probably want to use main health center at 526 29 ½ Road, because it offers medical, dental, behavioral health and pharmacy services. It also offers expanded hours of care on weekday evenings and Saturdays. All patients, regardless of which location they receive their care, can access the pharmacy at the main clinic.
What will happen to my care at Marillac if my economic or insurance status changes? (e.g., a new job with higher pay, loss of Medicaid)
If your economic situation changes, you should contact our Financial Assistance team to review your eligibility for our sliding fee discount scale. You can call (970) 200-1647 or (970) 200-1654 or email patient.eligiblity@primehealthplus.org Whether or not you still qualify for the discount, we’re happy to continue providing your care.
Do I need to confirm my appointment?
Yes. PH+ offers several options to confirm or cancel your appointment. We send reminder texts or calls before your appointment, or you may call us to confirm/cancel. You may also confirm/cancel via our patient portal.

You are required to confirm or cancel by noon on the business day before your appointment. If you do not, your appointment will be given to another patient, and you may have a “No Show” note added to your record. After three no-shows in any department, you may be limited on how you can access services.
What do I do if I can’t attend my appointment?
Life happens, and plans change! As soon as you realize you won’t be able to attend your appointment or you may be late, please contact us at (970) 200-1600 (GJ) or (970) 200-1674 (Montrose) to reschedule.
Do you provide Urgent Care services?
Yes. PH+ offers our patients urgent care services (we call it Convenient Care) by appointment or on a walk-in basis at our main site, 526 29 ½ Road, Monday-Saturday, with extended hours. Convenient Care needs might include fever, cough/cold, flu symptoms, bug bite, minor cuts or injuries, mild infections, etc. Our pharmacy may also be open Monday-Saturday for any prescriptions given during a Convenient Care visit.
Who can use the pharmacy, and what services does it offer?

PH+ offers a walk-in/drive-up pharmacy at our 526 29 ½ Road location. The pharmacy is open to everybody, even if you are not a PH+ patient. Our pharmacy participates in the 340b discount program, which makes prescriptions more affordable for those who qualify. We also take insurance and welcome discount cards like GoodRx.
